All residential players require a AAA PTS form in order to attend evaluations this spring. Please note as per 2022 OMHA AGM players are required to attend their home zone tryouts prior to attending evaluations in another centre. Some exceptions apply as per OHF Regulations (see E38)
Players rostered to Credit River for the 2023-2024 season DO NOT require a PTS for tryouts.
U18:PTS_U18_as_of_Aug_23
Permission to skate forms outside of Credit River will be available TBE for evaluations after August 2024. If you are a resident of the Credit River Capitals Zone PTS is required for Spring and they are for residential/zone players only (or previously rostered in 2023/2024) See below for additional information.
Eligibility to attend Spring U18 AAA Evaluations:
Skates prior to Aug 2024 are for in zone residential players only as per OHF AAA Regulations K.

OJHL/Jr Camps: FORM HEREPTS_Jr
Junior PTS forms are required for OMHA players wishing to attend Junior Hockey Evaluation/Camps. If your player was rostered to A U18 AAA team in 2023 2024 you are required to have the last rostered team sign this form.
Players/goalies who were rostered in Orangeville in 2023/2024 will require a signed form from OMH.
Please ensure the following in order to receive a signed PTS form:
- You have registered with Orangeville Minor Hockey (registration payment is not required until after AAA tryouts are completed payment plan commitments will be required prior to the AA tryouts commence)
- You complete the online PTS request form and provide full information required
