I, the interested candidate, hereby agree that if selected I will conduct my duties as a Coach with The Orangeville Minor Hockey Association under the umbrella of The Ontario Minor Hockey Association, The Ontario Hockey Federation and Hockey Canada appropriately and will adhere to all policies, procedures, fair play regulations and codes of conduct at all times.
